Please sign and return the following documents to ensure your enrollment will be processed in an expeditious manner:

v     Yellow Diocesan Enrollment Form (must be obtained from school office)

v     Enrollment Form

v     Tuition and Financial Agreement

v     Share Hours Contract

v     Smart Enrollment Form

Along with your completed application, please include the following documents and fees:

v     Birth Certificate

v     Immunization Records

v     Baptismal Certificate (if Catholic)

v     Previous School Records

v     Registration fee

You may retain for your records:

v     All additional Tuition and Financial Information

v     All additional Share Hour Information

v     Immunization Requirement Sheet

v     Entrance/Exit Map (obtained from school office)

v     Medication Form (for future use)

v     Total Learning Calendar

v     School Supply List

v     Tentative Calendar

(Families with preschool students will be charged an additional $250 in tuition fees.)
